BackgroundA number of region-specific validated triage systems exist; however very little is known about their performance in resource limited settings. We compare the local triage tool and internationally validated tools among under-fives presenting to an urban emergency department in Tanzania.MethodologyProspective descriptive study of consecutive under-fives seen at Muhimbili National Hospital (MNH), ED between November 2017 to April 2018. Patients were triaged according to Local Triage System (LTS), and the information collected were used to assign acuities in the other triage scales: Canadian Triage and Acuity Scale (CTAS), Australasian Triage Scale (ATS), Manchester Triage Scale (MTS) and South African Triage Scale (SATS). Patients were then followed up to determine disposition and 24h outcome. Sensitivity, specificity, positive and negative predictive values for admission and mortality were then calculated.ResultsA total of 384 paediatric patients were enrolled, their median age was 17months (IQR 7-36months). Using LTS, 67(17.4%) patients were triaged in level one, 291(75.8%) level 2 and 26 (6.8%) in level 3 categories. Overall admission rate was 59.6% and at 24h there were five deaths (1.3%). Using Level 1 in LTS, and Levels 1 and 2 in other systems, sensitivity and specificity for admission for all triage scales ranged between 27.1-28.4% and 95.4-98% respectively, (PPV 90.3-95.3%, NPV 47.1-47.4%). Sensitivity for mortality was 80% for LTS, and 100% for the other scales, while specificity was low, yielding a PPV for all scales between 6.9 and 8%.ConclusionAll triage scales showed poor ability to predict need for admission, however all triage scales except LTS predicted mortality. The test characteristics for the other scales were similar. Future studies should focus on determining the reliability and validity of each of these triage tools in our setting.

Developing efficient and recyclable heterogeneous catalysts for organic reactions in water is important for the sustainable development of chemical industry. In this work, Pd nanoparticles supported on DABCO-functionalized porous organic polymer was successfully prepared through an easy copolymerization and successive immobilization method. Characterization results indicated that the prepared catalyst featured big surface area, hierarchical porous structure, and excellent surface amphiphilicity. We demonstrated the use of this amphiphilic catalyst in two case reactions, i.e. the aqueous hydrodechlorination and Suzuki-Miyaura coupling reactions. Under mild reaction conditions, the catalyst showed high catalytic activities for the two reactions. In addition, the catalyst could be easily recovered and reused for several times. Also, no obvious Pd leaching and aggregation of Pd nanoparticles occurred up during the consecutive reactions.

A series of unsymmetrical 1,3-disubstituted benzimidazolium chlorides were synthesized as N-heterocyclic carbene (NHC) precursors. These compounds were used to synthesize of the PEPPSI-type palladium NHC complexes. The structures of all compounds were characterized by H-1 NMR, C-13 NMR, FT-IR spectroscopy and elemental analyses. The catalytic activity of the PEPPSI-type palladium-NHC complexes has been evaluated with respect to the Suzuki-Miyaura cross-coupling reactions of phenyl boronic acid with various aryl halides in aqueous media. An efficient synthesis of 3-functionalized coumarins is demonstrated fromo-cresols and active methylene compounds under solvent-, metal- and catalyst-free conditions in the presence of 70 % aqueoustert-butyl hydrogen peroxide. A highly efficient palladium-free homogeneous catalyst involving Ni(acac)(2)/2,6-bis(diphenylphosphino) pyridine ((Ph2P)(2)py)/CuI components was used for the Sonogashira cross-coupling reaction. The Sonogashira reaction was investigated between phenylacetylene and various bromoand chloroarenes containing electron neutral, electron-rich, electron-poor, electron-deficient, and sterically hindered aryl fragments. The aryl alkynes coupling products were obtained with good to excellent yields at the optimized conditions using Ni(acac)(2) (0.3 mol%)/(Ph2P)(2)py (0.6 mol%)/CuI (0.03 mol%) as the catalyst, tetrabutylammonium bromide (TBAB) as the additive, Et3N as the base in DMF at 100 degrees C under N-2 atmosphere.

A novel third-nonlinear optical (NLO) crystal, piperazine-1,4-diium bis(2,4-dichlorobenzoate) (abbreviated as PBDCB) was successfully synthesized and grown good quality crystal by slow evaporation solution growth technique by methanol as a solvent at room temperature. The formation of the grown crystal was carried out by single-crystal X-ray diffraction analysis. The analysis of a single crystal X-ray diffraction reveals that the title compound was crystallized in the monoclinic system with centrosymmetric space group P21/c and thereof lattice parameter are a = 11.166(6) angstrom, b = 8.160(5) angstrom, c = 12.085(6) angstrom, beta= 113.18(3) degrees, Z = 4. Crystallographic studies also reveal that the molecules are linked via N-HO and C-H Cl hydrogen bonds. Pairs of intermolecular N-HO hydrogen bonds generate R 4 4 (12) ring motifs. The powder X-ray diffraction study was completed and the peaks were indexed. UV-Vis spectral studies were revealed that the grown crystal is transparent in the entire visible region with a lower cut off the wavelength of 286 nm. The optical band gap of the title compound was measured by Tauc’s plot method, and it was obtained to be 3.9 eV, which shows suitable applications of the optoelectronic device. In the photoluminescence study, the grown crystal was revealed violet emission radiation, and thereof peak has appeared at 312 nm. The functional groups of the grown crystal have been determined by Fourier Transform Infra-Red (FTIR) spectroscopy technique. In thermogravimetric (TGA) and differential thermal analysis (DTA) analysis, the title material was determined as thermally as stable up to 163 degrees C. The Vickers Microhardness test ascertained the mechanical properties of the grown crystal material, and it belongs to a soft material category. The dielectric measurements of the developed crystal material were also ascertained at various frequencies. The dielectric constant, dielectric loss, and A.C. the conductivity of title material was studied with different frequencies and temperatures. The laser damage threshold (LDT) of the grown crystal was then calculated by using Nd: YAG laser. The grown crystal has given the measured (LDT) value of 8.97 GW/cm2. The third-order nonlinear optical (NLO) properties characteristics, such as absorption coefficient (beta similar to 0.02 x 10 -4 cm/W), the refractive index (n(2) similar to 4.06 x 10(-8) cm(2)/W), and their susceptibility range values of (chi((3)) similar to 2.17 x 10(-6)esu) of the grown crystal was estimated by Z-scan technique, suggests that the grown crystal material PBDCB was served as a promising source for nonlinear optical devices. The challenging nickel-catalyzed mono-alpha-arylation of acetone with aryl chlorides, pivalates, and carbamates has been achieved for the first time. A nickel/Josiphos-based catalytic system is shown to feature unique catalytic behavior, allowing the highly selective formation of the desired mono-alpha-arylated acetone. The developed methodology was applied to a variety of (hetero)aryl chlorides including biologically relevant derivatives. The methodology has been extended to the unprecedented coupling of acetone with phenol derivatives. Mechanistic studies allowed the isolation and characterization of key Ni(0)and Ni(II)catalytic intermediates. The Josiphos ligand is shown to play a key role in the stabilization of Ni(II)intermediates to allow a Ni-0/Ni(II)catalytic pathway. Mechanistic understanding was then leveraged to improve the protocol using an air-stable Ni(II)pre-catalyst.

A green and efficient method for the synthesis of amides has been developed through oxidative amidation between methylarenes with amines in a two-step continuous flow system. This method integrates methylarene oxidation and amide formation into a single operation which is usually accomplished separately. Oxidation with tert-butyl hydroperoxide (TBHP) as green oxidant, the synthesis of amides under mild reaction conditions in continuous flow system and the utilization of methylarenes as starting material make this methodology novel and environment friendly. An operationally simple nickel-catalyzed hydroarylation reaction for alkynes is described. This three component coupling reaction utilizes commercially available alkynes and aryl bromides, along with water and Zn. An air-stable and easily synthesized Ni(II) precatalyst is the only entity used in the reaction that is not commercially available. This reductive cross-coupling reaction displays a fairly unusual anti selectivity when aryl bromides with ortho substituents are used. In addition to optimization data and a preliminary substrate scope, complementary experiments including deuterium labeling studies are used to provide a tentative catalytic mechanism. We believe this report should inspire and inform other Ni-catalyzed carbofunctionalization reactions.

A novel oxidative esterification of 1-arylprop-2-en-1-ols with toluene derivatives catalyzed by tetrabutylammonium iodide (TBAI) is reported. The optimization of the reaction conditions illustrates that each of experiment parameters including the catalyst, solvent, and oxidant is significant for present oxidative functionalization. This metal-free protocol has a broad substrate scope including the halogen groups for further functionalization and enriches the reactivity profile of allyl alcohol and toluene derivatives. In addition, this protocol represents a new transformation of allyl alcohol involving C-C bond cleavage and C-O bond forming.
